Job Description
The Home Care Registered Nurse (RN) is responsible for delivering comprehensive, autonomous clinical nursing care directly to patients within their residential environments. The role ensures the successful execution of specialized physical assessments, wound management routines, and intravenous therapies while promoting patient safety, independent wellness, and stellar documentation compliance.
Required Qualifications * Associate or Bachelor’s degree in Nursing (BSN preferred) from an approved nursing academy.

Active, valid Registered Nurse (RN) license within the state of practice.

Active driver's license with a clean driving record and reliable personal transportation.

Responsibilities Duties:
Key Responsibilities * Coordinate and execute personalized home care plans, driving medication reconciliations and physical therapy coordination.

Prepare accurate, real-time clinical notes, wound progression photos, and OASIS assessment documentation.

Administer prescribed medications, complex dressings, injections, and phlebotomy services according to physician orders.

Coordinate with primary care physicians, clinical supervisors, family caregivers, and community healthcare aids.

Monitor changes in patient vital indicators and promptly address signs of physiological decline or home safety concerns.

Maintain clean infection control safeguards and appropriate equipment tracking during field travel.
