Job Description
The RN Manager of Care Management is responsible for overseeing the day-to-day clinical operations, social service allocations, and continuum-of-care programs within the facility. The role ensures the successful execution of daily patient assessments, transition plans, and utilization reviews by mentoring and guiding frontline nurse case managers and clinical social workers.
Required Qualifications * Bachelor’s degree in Nursing (BSN) from an approved healthcare training track.

Active, unencumbered Registered Nurse (RN) License in the state of California.

3+ years of clinical acute-care case management experience coupled with introductory leadership experience.
Preferred Skills * Strong team engagement, clinical mentoring, and patient advocacy talents.

High organizational dexterity to successfully manage multiple complex medical discharges under tight schedules.
Responsibilities Duties:
Key Responsibilities * Coordinate and execute frontline staff nursing assignments, daily case evaluations, and cross-shift reporting.

Prepare staff productivity evaluations, clinical educational programs, and complex discharge pathway profiles.

Monitor daily admission criteria checking for patient placement accuracy and appropriate care tracking.

Coordinate with multidisciplinary medical teams, post-acute care facilities, and patient immediate support systems.

Monitor ongoing case bottlenecks and promptly address operational friction points or unaligned care practices.

Maintain clean compliance records regarding utilization documentation guidelines and hospital policies.
