Job Description
The Nurse Practitioner (Primary Care & Senior Services) is responsible for delivering comprehensive diagnostic, therapeutic, and preventative healthcare services explicitly focused on adult and geriatric patient panels. The role ensures the successful execution of chronic illness management, preventative wellness mappings, and cognitive evaluations while focusing on maximizing quality of life, patient safety, and clinical coordination.
Required Qualifications * Master’s or Doctoral degree in Advanced Practice Nursing (MSN/DNP) from an accredited institutional program.

Active, completely unrestricted Advanced Practice Registered Nurse (APRN) or Nurse Practitioner license in Virginia.

National Board Certification as an Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) or Family Nurse Practitioner (FNP).
Preferred Skills * Deep structural command over geriatric syndromes, cognitive decline metrics, and modern neuro-pharmacology networks.

Exceptional communicative empathy and structural capability to direct patient health literacy.
Responsibilities Duties:
Key Responsibilities * Coordinate and execute comprehensive geriatric physical assessments, complex diagnostic screenings, and therapeutic plans.

Prepare detailed treatment blueprints, ordering labs, dynamic radiographic scans, and coordinating multi-specialty referrals.

Prescribe pharmacologic therapeutics, controlled substances, and medical equipment devices according to state codes.

Coordinate with collaborating primary physicians, home care nurse managers, families, and community social services.

Monitor patient medication tolerances and polypharmacy metrics, promptly adjusting treatment protocols to limit adverse reactions.

Maintain absolute accuracy within Electronic Health Records (EHR), satisfying all documentation guidelines.
